NHS Kernow is the Clinical Commissioning Group for Cornwall and the Isles of Scilly. The CCG are undertaking a number of service improvement projects to improve care for patients with long-term conditions, including diabetes. In particular they are reviewing their foot care pathways with the aim of reducing their amputation rates.
Diabetes foot care in Cornwall
In April we jointly hosted a series of three focus groups with local people with diabetes, to capture their experiences of foot care services in order to influence the review of the care pathway. The CCG were also keen to hear from patients about how they can improve education about foot care in the area, and other ways support people with diabetes to look after their own feet better.
