Hardwick CCG in North East Derbyshire has a relatively high rate of prevalence of diabetes and faces challenges in terms of improving performance against key outcomes areas. They are aiming to help people with diabetes to improve the self-management of their condition as well as encourage early diagnosis of diabetes by increasing take-up of NHS Health Checks.
Self-management and diagnosis
Diabetes UK jointly hosted an engagement event with Hardwick CCG to involve local people with diabetes. The session, held at Chesterfield FC’s Proact Stadium, gave service users the opportunity to tell Hardwick CCG their opinions on their current diabetes care, including their experience on diagnosis and how they could be helped to self-manage their condition. The 40 or so people with diabetes that attended the event also had the opportunity to ask a panel of specialists questions about diabetes care in Hardwick. The feedback gathered will be used to shape the focus of the CCG’s service improvement projects.
