As one of our Clinical Champions, Dr Fayers wishes to build the pathway evidence base for an effective outreach strategy for primary care, with a view to improving patient outcomes across local STPs. Central to this will be the development of a community centred Type 1 diabetes pathway and community access to Type 1 technology.
Background
Dr Fayers has led the West Hampshire Community Diabetes Service as a Consultant Diabetologist since her appointment in 2010 at the launch of the new service which replaced traditional hospital based care.
During that time Dr Fayers reached outside the commissioned contract to develop a series of quality improvement initiatives that embed the community service with healthcare professionals and patients across the entire trust, including projects that have been shortlisted for national awards (BMJ and HSJ).
Dr Fayers believes that effective leadership can nurture a team culture that supports individuals to develop their ambitions and autonomy and should not depend on one individual. Delivering more effective, higher quality care without additional funding is the key to a sustainable diabetes pathway. In this way she has worked with senior leaders to re-commission the diabetes service based on the needs of the local patient population and more recently to deliver a rapid transformation project that reaches across organisational and county boundaries to improve the care and treatment target outcomes for people with diabetes.
