Medicines Optimisation Pharmacist, NHS Leeds North CCG
Clair is a CCG based pharmacist, who leads on improving the quality of diabetes prescribing in general practices across North Leeds CCG. She has led on the development of the Leeds Prescribing Guidance for Type 2 diabetes, which is used widely both in hospital and general practice.
Clair has been practising as a pharmacist for 22 years, in both hospital and primary care. Clair first developed a passion for diabetes 15 years ago, when working in a hospital diabetic clinic managing patient’s cardiovascular risk factors.Â
Clair is an active member of the local diabetes network. She has recently participated in a system-wide quality improvement programme in diabetes, run by the Leeds Institute of Quality Healthcare (LIQH). Clair worked as part of a multi-disciplinary team looking at improving the uptake and completion of structured education across the city, as well as being responsible for the many changes in the diabetes element of the CCG prescribing engagement scheme which has resulted in significant changes in prescribing patterns and improved patient care across the CCG.
Clair’s main goal as Clinical Champion is to devise minimum standards for insulin prescribing in general practice, exploring the role the patient and community pharmacists could play in helping to implement these standards.Â
